Infobox disease template

I worked on http://en.citizendium.org/wiki/Template:Infobox_Disease. Please make further changes or suggestions. Note that I intentionally left out links to DiseasesDB and eMedicine as I believe these to be either too commercial (eMedicine), or too dependent on the work of one person (DiseasesDB). I think eMedicine is not good enough and transparent enough to link to and I think DiseaseDB's future is too dependent on one person who maintains it (see http://www.diseasesdatabase.com/money.asp).
